Fifth Avenue Entertainment Membership FAQ
How
much will the membership cost to purchase?
The membership will be sold for $15

The membership will be sold in June 2004 and will be active

How
long will the membership be valid?
The membership will be valid for 1 year.
What
type of establishments will list on the membership?
There will be restaurants, comedy clubs, theatres and pubs
What is a one time use offer?
A one
time use offer is a promotion provided by a participating
establishment to be used by a membership holder one time.
For a one time use offer, a number will be printed on the back of
the 5th Avenue Entertainment membership card corresponding to the
participating establishment. The number will be marked off
by the participating establishment when the offer is used.
What is a reusable offer?
A
reusable offer is a promotion provided
by a participating establishment
to be used by a member holder for the entire
year. The membership card holder can use the membership card
at participating establishments as many times as they would like.
If an offer is reusable, the word reusable will be listed
with the offer.
What is
an offer number?
An
offer number corresponds to a participating establishment
providing a one time use promotion to 5th Avenue Entertainment
membership card holders. The offer number is printed on the
back of the membership card and will be marked off once the card
is used at the participating establishment.
How many membership cards can be used when I dine in groups?
When
dining in groups you can use up to two memberships cards
when the offer is specific in nature (i.e. Buy-one-get-one) unless
stated otherwise by participating establishments. When the
offer is a percent in nature, you may use only one card per
table. The percentage will be deducted from the entire bill
for the table (excluding alcohol) unless stated otherwise by
participating establishments.
Are there any days that the membership card cannot be used?
Yes,
your membership card cannot be used during any of the following
days unless stated otherwise by participating establishments:
Christmas Eve/Day, New Years Eve/Day, Valentine’s Day,
Mothers/Fathers Day, Thanksgiving, Easter, Good Friday, Labour
Day, Canada Day, Boxing Day, Remembrance Day, other Civic holidays
observed by corresponding establishments.
Do I need
to register my membership to receive the
promotions from the listed establishments?
No.
You do not need to activate your membership to receive any offers
or promotions from the listed establishments. However, in
order to leave feedback and to be eligible to win gift
certificates for participating restaurants, you must register your
membership.
Why should I register my Fifth Avenue Entertainment
membership?
Registering your Fifth Avenue Entertainment membership will allow
you to leave feedback on your visit to participating restaurants.
It will also allow you to be eligible to win gift certificates for
participating restaurants as we will continually have draws to win
gift certificates.
Does Fifth Avenue Entertainment share any of my information
with outside sources?
No.
We will never share your information with outside sources.
In the case of negative feedback being entered for a participating
restaurant by a Fifth Avenue Entertainment membership holder, the
restaurant owner may request the information of that person who
did not receive proper service. If this case does arise, the
membership holder will be contacted by Fifth Avenue Entertainment
for approval to release their information.
How
does the feedback system work?
The feedback system allows members to leave feedback on their
visit to a participating establishment. Feedback for participating
establishments will be viewable to all members.
Why is leaving feedback so important for membership holders?
Leaving feedback will allow other membership holders to read about
your experience at the participating restaurant. The
feedback will give other membership holders a better idea of
quality, service, and the overall appeal of the restaurant.
Why is leaving feedback so important for
the restaurant owner?
Leaving feedback, both positive and negative, is very important to
the restaurant owner. Feedback gives the owner a chance to
see how they are performing in quality, service, and overall
appeal to those visiting their restaurant. Feedback gives
the owner a chance to see where they need to improve and where
they have succeeded.
When I
leave feedback on my visit to a participating restaurant, does the
feedback automatically appear on the system for everyone to view?
No.
The restaurant owner is given a chance to view the feedback before
it is viewable to all our members. This will allow any
negative feedback to be addressed immediately, and allow the
negative experience to be corrected. After the owner of the
participating restaurant has viewed the feedback, it will then be
publicly available.
Will
Fifth Avenue Entertainment promote the use of the feedback system?
Yes, we will promote the use of the feedback system by having
draws to win gift certificates for participating establishments.

What
is a Client Membership?
A
client membership is a reusable membership that is provided to
all clients of Fifth Avenue Entertainment. The client membership
will allow establishment owners (clients) to visit those participating
establishments listed on the membership.
How
does the Client Membership Work?
The client membership allows clients to visit participating establishments
of Fifth Avenue Entertainment. The client membership is reusable
so clients can use the membership as much as possible. All clients
using their client membership are required to leave feedback on
the member feedback system.
What
is the Member Feedback System?
The
member feedback system is designed to allow those clients using
their reusable client membership the ability to make comments
on the establishment visited. All feedback left on the member
feedback system is private, therefore password protected.
How
do I Access the Member Feedback System?
A
password will be provided to all clients enabling access to the
system.
